2015 Health, Permission and Travel Forms


1. Medical Form for day and overnight camps. BSA Annual Health and Medical Record: This form is required for all participants, youth and adult, attending Glacier’s Edge or other Scout Camps. Part A(Health History) and Part B (parent/guardian consent form) must be completed and a copy provided for each camp attending. Parents and/or guardians/adults attending the camps must also complete the forms.
We suggest that you fill out a form (for scouts and parents) without dating it, and make copies so you don't have to fill a new one out for each event.

You can get immunization records for your child if the immunizations were given in the State of Wisconsin here: (Wisconsin Immunization Registry)

2. Medication Form.  If you have a scout or parent attending any Glacier's Edge camps that will need to take any medication, the following consent form must be filled out.

3. Vehicle Form  - Should be completed and kept on file for all Scout-related trips.

About Pack 549

Pack 549 serves approximately 40-50 of Verona's finest future citizens through the Boys Scouts of America's Cub Scouting program. Pack 549 is part of Glaciers Edge Council. The boys range from ages 6 through 11, and work through age-appropriate rank activities in smaller groups called Dens to attain rank as Tiger, Wolf, Bear, Webelos I and Webelos II scouts. Pack meetings are generally held on the 3rd Thursday of each month at Glacier Edge Elementary School in Verona, at 6:30 pm. Scouts are awarded rank advancement and major awards at these meetings. In addition Pack to meetings, each scout meets with his Den at least once, and sometimes more frequently during the month, to work on achievements and electives to earn rank. Advancement toward rank is recognized in the Den meetings. Boys need to wear their Class A uniform at pack meetings. They are encouraged to wear either Class A or Class B uniforms, as appropriate to their Den meetings (Class B uniforms are Verona Scouts 'T' shirt purchased from the Pack). All scout units are chartered by a supporting organization. Pack 549 is chartered by the Salem United Church of Christ, Verona, WI at Salem United Church of Christ. Join Cub Scouts! Class B Uniforms - T-Shirts for Pack 549

Classs B Uniforms consist of T-Shirts with "Pack 549" printed on the shirt.  The cost is $10.00 and can be ordered at most Pack Meetings.  Adult shirts can also be ordered for $12.00.  The shirts are a special poly blend that "breathe" and stay dry and cool for the summer camps and outdoor activities.

Class "A" uniforms are worn at Pack and Den meetings.  Class "B" or t-shirts are worn for less informal activities.  The leader of the event/camp will inform the Scout when the shirt will be worn for the specific event and will be specified in the event/camp notice.

Shirts are available in all sizes for the Scout, Den Leader and parent.

Provides summary of each rank's requirements, belt loops, ideas for meetings. Pack Meetings at Glacier Edge

Glacier Edge School off of Whalen Road is the new location for the monthly pack meetings.
Glacier Edge offers a nice stage for skits and speaking, has a built in P.A. system, screen and projector. The map to the right will take you to a Google map showing the school location.
Pack meetings start at 6:30 pm. The meetings are held the third Thursday of the month during the school year.  Summer meetings/activities are held at other various locations.

Bobcat Badge Worksheet

This worksheet could be used in a den meeting at home for scouts working on their Bobcat badge.
Bobcat is the first rank each scout must obtain, and then they start to work on the same badge as the rest of the den. So a new scout joining in Bear year will first work on the Bobcat, then on the Bear. Skipping the Tiger and Wolf.

Pack's Parent Guide and Interest Survey

1 - We would like to Provide the Parent Guide for New Families and also for everyone's review. If you are a New Family and you did not get a Pack 549 Shoulder Patch let me know. We added 18 new scouts... so welcome our two new Tiger Dens (6 and 7) and the Wolves and Bears that joined active Dens. We will also have a fall recruitment event. A parent interest survey is to be completed and given to your Den Leader and Pack Trainer . The Pack will then have a copy for planning.
